FAILURE TO SUBMIT YOUR HEALTH FORMS WILL JEOPARDIZE YOUR ENROLLMENT!
All students enrolled in classes at the Utica University must submit proof of MMR vaccines, Meningitis vaccines or a waiver as well as COVID-19 vaccines and COVID-19 boosters.
We must have your immunization records by July 1st, 2022. You will NOT be able to move onto campus or attend classes if we do not have your immunization records on file when classes begin! Immunization records can be obtained from your primary physician, high school, or any other college you have attended.
Return your Health Form by uploading the forms in the secure Student Health Portal, by mail, or by fax. The return address and fax number are listed on the top of the form. Student Health Portal The Student Health Portal is a convenient way to submit your health forms. The Student Health Portal is a secure, confidential feature students can use to communicate with the Student Health Center, and manage their medical information. Click the box below to create your secure health portal account using your nine digit Utica University ID number, fill out the immunization form on your portal site, and then upload your health information.
Health Insurance All full-time students must have health insurance, and the University offers an affordable student plan through United Healthcare. If you are covered through another health insurance policy, you can waive the fee beginning June 7, 2022.
