Name Changes


To change your name, you must submit the legal documentation to the Registrar's Office with the official declaration of your name change (e.g., copy of new driver's license, marriage license, divorce decree, or other legal document).

Please note that name changes done in the Registrar's Office only changes your name in the Banner system. If you wish to change your e-mail address, you must e-mail the Help Desk at with your old and new names and your telephone number in order to initiate this change. These requests are usually processed between semesters (not while classes are in session) so as not to cause confusion mid-term.

Please ensure that your instructors are aware of your name change issue.

LOCAL ADDRESS - For students who do not live locally year-round, this is the address where you reside during the school year.
DIPLOMA MAILING ADDRESS - Only fill out this portion if you have already submitted an application for a degree.
